Athletic Physicals

The ATHLETIC PHYSICAL FORM can be picked up in the Badger Front Office.

A copy of that physical needs to be turned in to the Badger Front Office or uploaded during the online registration process.  If your physical is in your current school's nurse’s office, you must get a copy of that physical and turn it in to the Badger Front Office or upload it during the online registration process. 

Physicals after 4/1/2025 are good until 6/30/2027
Physicals after 4/1/2026 are good until 6/30/2028
Physicals after 4/1/2027 are good until 6/30/2029

Turn in a copy of this physical to the Badger Athletic Office.  This physical must be on file with the Badger Athletic Office prior to participating in athletics.
