Athletic Physicals

The ATHLETIC PHYSICAL FORM or can be picked up in the Badger Front Office.

A copy of that physical needs to be turned in to the Badger Front Office or uploaded during the online registration process.  If your physical is in your current schools nurse’s office you must get a copy of that physical and turn in to the Badger Front Office or upload during the online registration process. 

  • Physicals after 4/1/2022 are good until 8/1/2024
  • Physicals after 4/1/2023 are good until 8/1/2025
  • Physicals after 4/1/2024 are good until 8/1/2026

Turn in a copy of this physical to the Badger Athletic Office.  This physical must be on file with the Badger Athletic Office prior to participating in athletics.
